At Pathways Healthcare, we are thrilled to embark on our new journey into Hospice care. As we expand our services, we are looking for a compassionate and skilled On Call Hospice Triage Nurse to join our team.

Westwood & Boston areas - Job Type: Part-time
- Schedule:
Saturday and Sunday 8:30 am - 8:00 pm
- Initiates appropriate preventive and rehabilitative nursing procedures.
- Administers medications and treatments as prescribed by the physician.
- Initiates the plan of care and makes necessary revisions as patient status and needs change.
- Responds promptly to incoming calls during after hours. (Weekends 8:30 AM to 8:00 PM)
- Assesses patient’s needs through triaging and providing in-home care when needed.
- Collaborates with physicians, nurse practitioners and interdisciplinary team to contribute a strong Plan of Care for each patient.
- Responds to inquiries and concerns from patients, their caregivers, and their medical team.
- Consults patients and caregivers before, during and after treatment.
- 2+ years' nursing experience, preferably in geriatrics is desirable.
- Degree in Nursing from an accredited institution.
- Proficient with electronic medical records.
- Excellent bedside manner and communication skills.
- Working knowledge of health care laws and regulations.
- Maintain current BLS CPR certification.
- Possess a valid driver's license and own a reliable, insured vehicle.
- Pay Structure:
Base Compensation ($600 per weekend) + Paid per visit ($50) and per admission ($100). Visits over 60 minutes are paid at $75. Training and meetings are paid hourly at $35/hr. - Mileage Reimbursement ($0.36/mile)
