Job : 842

Shift: Monday-Thursday 8:30am-4:30pm, F 8:00am-4:00pm

Salary: $53,587.50

Essential Duties
  • Initiate and maintain contact with each family in the caseload, assessing needs and ensuring follow-through on program requirements.
  • Plan, coordinate and help manage the delivery of in-home behavioral services, purchase of goods and other essential services.
  • Maintain open lines of communication with DDS Clinical Managers and attend monthly meetings with DDS.
  • Develop the individual's person-centered Autism Support Plan and Emergency Fact Sheet.
  • Enter and maintain data in a designated consumer database.
  • Coordinate monthly meetings with each family on the caseload.
  • Assist family in hiring, scheduling and paying support workers, respite care and goods and services.
  • Maintain necessary records documenting work performed: travel, mileage, contacts made, and a database of individuals served.
  • Assist the Director and staff of the ASC in the planning, development, coordination, and management of services and supports to families.
  • Ability to work a flexible schedule to meet the needs of individuals and facilitate programs.
  • Excellent interpersonal skills.
  • Ability to multitask and prioritize tasks with attention to detail.
  • Ability to work in a team environment.
  • Daily travel in the Newton/South Norfolk & Metro-Boston area.
Requirements
  • Experience working with individuals with /DD.
  • BA/BS and experience in the Human Services Field, or Associate Degree plus 5 years’ experience working in the Human Services Field.
  • Must be able to pass fingerprint background check, CORI, DPPC and driving background checks.
  • Valid driver’s license required.
  • Excellent verbal and written communication skills.
