Hartford Healthcare - - - Responsibilities:
Provides personal hygiene and grooming care to clients; assists with toileting and ambulation; provides meals for clients' nutritional needs; reports changes in clients' condition to nurse, supervisor, or client care coordinator; performs routine housekeeping and home management duties; records each client's care on care plan and submits care plan in a timely manner; provides emotional support for clients and their families;
assists with routine therapy exercises; participates in case conferences as requested; attends all required in-service education sessions; uses computer software program and smartphone for required education; represents the company with a high level of customer service; can think critically without direct supervision and escalate when appropriate.
