The Family Peer Support Triage & Scheduling Coordinator serves as the first point of contact for families referred to the Family Peer Support Program. This role is responsible for contacting brief referrals, verifying information, determining service eligibility, and scheduling consultations with Family Peer Support Specialists (FPSS). The Coordinator must demonstrate professionalism, empathy, and strong communication skills while working with families during potentially stressful situations.
Key Responsibilities- Contact all brief referrals in a timely manner
- Serve as the initial point of contact for families seeking services
- Verify demographic and referral information
- Determine appropriate service pathway (Family Peer Support Services or Fast Track)
- Schedule consultations with the appropriate Family Peer Support Specialist based on geographic area
- Accurately document referral activity, including:
- Scheduled consultation dates
- Length of phone conversations
- Messages left or unsuccessful contact attempts
- Disconnected or non-working phone numbers
- Complete data entry as required
- Maintain accurate and complete electronic records
- Perform additional duties as assigned by the Director of Peer Support Services
- Strong working knowledge of Microsoft Outlook
- Ability to maintain accurate, complete, and organized electronic files
- Ability to prioritize tasks and work effectively under pressure
- Strong interpersonal skills and the ability to collaborate with diverse families, providers, and FAVOR staff
- Clear and effective verbal and written communication skills
- Bilingual speaking ability is a plus
