Job DescriptionWorks in coordination with interdisciplinary teams to organize, prepare, and deliver evidence based clinical services across the continuum of care. Maintains ethical principles and professional standards of practice in adherence to the discipline’s professional code of ethics.
Responsibilities- Perform clinical diagnosis in conjunction with conducting a comprehensive bio‑psychosocial assessment and establishment of a care plan.
- Champions integrated care model treatment guidelines in the primary care setting by providing approximately 6‑8 sessions of short‑term, focused treatment.
Minimum Requirements:
- Master’s Degree in Social Work, Counseling, Marriage and Family Therapy or a related human service field.
- Previous experience in a healthcare setting.
- 3‑5 years experience as a licensed clinician is preferred.
- LCSW, LPC, LMFT, LADC
- Valid Driver’s License
