HHCMG Referral Coordinator is responsible for new patient referrals being sent to the Primary Care Behavioral Health service line. Patients will be contacted and scheduled according to their primary care office location. Schedules will be reviewed daily to review for openings to get patients from the referral work queue or reschedule list scheduled sooner in order to ensure providers have a full schedule.
Each referral will be reviewed for completeness, ensure necessary documents are received and the referral meets the Behavioral Health guidelines for an appointment. Coordinator must properly communicate with front desk staff, providers, and managers in each office. Referral coordinators frequently interact with patients, clinicians, and managers. Coordinator will be expected to have experience with customer service tasks and the ability to maintain patient records and confidentiality, manage multiple schedules, and ensure that patient information is complete and up to date.
Experience:
Minimum of 1 year as a medical receptionist or previous care coordination experience
