Referral Coordinator - Endocrinology

Hartford Healthcare - 1290 Silas Deane Highway - Responsibilities:
Process new patient referrals to the Endocrinology service line, both internally and externally.;
Schedule patients based on primary care office location and daily openings to maintain full provider schedules.;
Review each referral for completeness and ensure required documents are received and compliant with Behavioral Health guidelines.;
Communicate effectively with front desk staff, providers, and managers across offices.;
Maintain patient records with confidentiality and support customer service interactions with patients, clinicians, and managers.
