Physical Therapist

Seeking licensed Physical Therapists to deliver personalized in-home and senior living care to older adults.

Responsibilities Include

• Providing one-on-one therapy sessions in private residences, senior living communities, and virtual settings.
• Performing evaluations, developing individualized treatment plans, and delivering evidence-based interventions.
• Supporting patient mobility, independence, and overall wellness.
• Documenting care accurately in electronic medical records.
• Collaborating with an interdisciplinary team of healthcare professionals.
• Educating patients, caregivers, and staff to promote safety and aging in place.
• Utilizing telehealth to extend care access and flexibility.

• Valid Physical Therapy license or eligibility in practice state.
• Degree from an accredited PT program.
• Proficiency with EMR systems and basic computer skills.
• Current CPR certification.

Ability to exert moderate effort, assist with patient transfers, reposition patients, and perform therapeutic activities, including driving to patient locations and wearing PPE as needed.
