Opportunity

Advanced Practitioners - Primary Care

Job Details
  • Seeking a full‑time Family Nurse Practitioner (FNP) or Physician Assistant (PA) to join a hybrid remote primary care team
  • Provide Transition of Care, Annual Wellness Visits, and same‑day primary care visits remotely
  • Scheduled on‑site time to support the clinical team and see patients virtually at Campus Care locations
  • Hybrid schedule: virtual/remote from home plus two days in person at Campus Care sites
  • Full‑time weekdays with occasional rotating weekends
  • No call schedule required
  • Work alongside a skilled and flexible team of Advanced Practitioners supporting growing virtual care services
  • Perform routine physical examinations and formulate medical, educational, and psychosocial treatment plans
  • Develop plans for further evaluation of health needs including referrals to specialists and care team members
  • Provide appropriate treatment within scope of practice per established protocols
  • Unrestricted State of Connecticut Medical License required (or eligible)
  • 5+ years of post‑graduate experience as an FNP or PA in a primary care setting required
