Oak Hill is seeking a highly motivated and dynamic Licensed Practical Nurse (LPN) to assist in maintaining and improving the health and wellbeing of individuals by monitoring health status and providing appropriate treatments to individuals with Developmental and Intellectual Disabilities to enable their highest quality of life in terms of their physical, emotional, and mental health while adhering to Oak Hill’s policies and procedures.

We are looking for a compassionate and skilled Licensed Practical Nurse (LPN) to join our team and provide essential nursing support to individuals with disabilities living in a residential group home setting.

$4,000 Sign-On Bonus! Sign-on bonus subject to applicable eligibility requirements and Oak Hill's bonus agreement.

As an LPN, you will play a critical role in supporting the health, safety, and well-being of the individuals we serve. Working collaboratively with Direct Support Professionals, program leadership, physicians, families, and interdisciplinary team members, you will:

• Provide direct nursing care and ongoing health monitoring for individuals living in a group home setting.
• Administer medications and treatments in accordance with physician orders and applicable regulations.
• Monitor, assess, and document changes in individuals' health status.
• Coordinate medical appointments, follow-up care, and communication with healthcare providers.
• Maintain accurate and timely clinical documentation and medication administration records.
• Support and educate residential staff on healthcare plans, medication protocols, and individual health needs.
• Respond appropriately to medical concerns and emergencies.
• Promote independence, dignity, choice, and the highest possible quality of life for every individual served.
• Collaborate with an interdisciplinary team to ensure coordinated, person-centered care.

The ideal candidate is a compassionate nurse who understands that exceptional healthcare is about more than clinical skills— it's about building trust, respecting individuality, and helping people thrive.

• Current Connecticut Licensed Practical Nurse (LPN) license in good standing.
• Experience working with individuals with intellectual and developmental disabilities is preferred but not required.
• Strong clinical assessment, communication, and documentation skills.
• Ability to work independently while collaborating effectively with a multidisciplinary team.
• A commitment to person-centered care, dignity, inclusion, and respect.
• Valid driver's license and ability to meet Oak Hill’s driving requirements, as applicable to the position.
