Position
Case Manager I
Shift and ScheduleFirst Shift, Full Time, 40 hours weekly – Monday to Friday, 7:00 AM to 3:00 PM.

Scope of ServiceThe Case Manager I provides comprehensive support focused on skill building and case management to help clients achieve independence and well‑being. Responsibilities include conducting screenings and assessments, developing person‑centered care plans, and delivering services through a team approach, including rehabilitative supports, skill building for activities of daily living, access to resources, facilitating psycho‑education groups, socialization, and community integration. The role also involves completing documentation per contract and agency requirements, maintaining a clean work environment, and performing other duties as assigned.
EssentialJob Functions
- Establish rapport and maintain professional ethics and boundaries with clients.
- Enhance clients’ daily living skills using varied teaching modalities (modeling, prompting, hands‑on assistance).
- Develop, implement, and monitor person‑centered plans, including intake and discharge processes.
- Coordinate case‑management services, including referrals, entitlements, money management, medical care, transportation, housing, wellness, legal services, education, and employment.
- Identify barriers to clients’ aspirations, link them to appropriate services, and review progress regularly.
- Support clients in learning transportation skills and provide transportation as needed; staff may transport clients in personal vehicles within community settings and shared‑living programs.
- Conduct apartment/room inspections and assist clients in maintaining clean living spaces.
- Build relationships with families, natural supports, and external providers; advocate on behalf of clients when necessary.
- Facilitate/co‑facilitate psycho‑educational groups as indicated.
- Complete service entry documentation per contract and agency requirements.
- Administer breathalyzer tests and urinalysis samples as applicable.
- Conduct rounds within the program and participate in staff meetings, All Treater’s meetings, rounds, and supervision.
- Bachelor’s degree preferred; minimum of one year experience. High school diploma considered with two years’ experience, including one year working with Continuum.
- Prior experience working with people with persistent chronic mental illness and/or addiction.
- Valid driver’s license and reliable vehicle required for community‑based and shared‑living programs; minimum CT State liability insurance required.
- CPR/First Aid certification upon orientation; ongoing certification required.
- Proficient reading, writing, and oral communication, including computer proficiency and familiarity with electronic record keeping.
- Knowledge of community support systems and resources preferred.
