Under the general guidance from the Executive Director of APCs with medical oversight from the MIH Physician leadership, the Physician Assistant or Nurse Practitioner will provide telehealth care to patients in the community. Rapidly growing service line adding provider coverage to meet the growing need in the community. the NP/PA will evaluate, diagnose and treat patients following written guidelines approved by the Department of Medicine.

Essential FunctionsPatient remains in MIH Telehealth program – schedule Telehealth visit for next day (remains on list);
Patient required MIH Paramedic visit – Provider to place order and coordinate with MIH Administration;
Patient follows ups with PCP – Provider to outreach to PCP and coordinate PCP to follow patient;
Patient required VNA services – Provider to place order and coordinate with VNA;
Patient requires Medical Control oversight – Provider to outreach to MIH Medical Control
Patient discharged from program – need to collect device;
Patient remains in MIH Telehealth program – schedule Telehealth visit for next day;
Patient requires second MIH paramedic visit – provider to coordinate with MIH Administration;
Patient follows ups with PCP – Provider to outreach to PCP and coordinate PCP to follow patient;
Patient requires VNA services – Provider to place order and coordinate with VNA;
Patient requires Medical Control oversight – Provider to outreach to MIH Medical Control

Shifts and Work ScheduleThree 12 hour shifts per week. Rotating 8a-8p and 11a-11P

License/Registration/Certification Requirements: ACLS (AHA) Advanced Cardiac Life Support Certification - American Heart Association (AHA) (Including courses offered through SSH), Certified Nurse Practitioner - Board of Registration in Nursing (Massachusetts)
