Overview

Care One at Weymouth, in partnership with Bridge Care Provider Group, is hiring an Internal Medicine Nurse Practitioner. Salary range: $125,000 to $135,000 annually. Compensation based on experience, qualifications, credentials and other relevant information.

Responsibilities
  • Conduct patient visits in accordance with local, state, and federal regulations.
  • Communicate visit findings to designated members of the interdisciplinary team.
  • Facilitate access to appropriate support models of care (e.g., rehabilitation, ongoing aggressive treatment).
  • Consult with other members of an interdisciplinary team to contribute to the patient’s plan of care.
  • Prepare and maintain accurate patient records, charts, and documents.
  • Assist in development of clinical practice guidelines/standards in support of quality care as needed.
  • Ensure collaboration with patients, caregivers, medical directors, and members of IDT.
Education

Master’s degree in Nursing (MSN) or Doctor of Nursing Practice (DNP) from an accredited program.

Professional Experience
  • Must hold a Nurse Practitioner’s license in the state of operations.
  • 1+ year experience working within Family, General Practice, Geriatric care or a related field.
  • Valid driver’s license and reliable transportation.
  • Willingness to travel locally.
  • Experience in a nursing home, hospital, assisted living or acute rehab space is a plus but not required.
  • Commitment to ongoing professional development and clinical excellence.
