Nurse Practitioner

Currently seeking a Nurse Practitioner to join a team full time at a Hospital at Home Program Under the supervision of the Hospital at Home (HAH) Medical Director the nurse practitioner will perform/supervise services according to the policies, procedures, philosophy, and objectives of the department and hospital.

Virtual patient visits and/or in-person home visits

Will share on-call responsibilities with the Physician/Medical Director

Responsibilities include:

  • Complete admission physical exams accurately, thoroughly, and in a timely manner utilizing appropriate growth and development parameters
  • Assess patient/family psychosocial needs, educational needs, learning readiness, and barriers to learning during admission process.
  • Provide patient education for the patient and/or family consistent with assessed needs.
  • Collaborate with other disciplines in planning and coordinating patient care. Allow for optimal patient progress toward wellness and healthy lifestyle.
  • Prescribe and comply with NP protocols and Massachusetts DPH regulations regarding prescriptive practices.
  • Participate in daily huddles with other team members, including RN, MIH paramedic and physician.
  • Collaborate with other disciplines (including but not limited to pharmacy, case management and therapists) in planning and coordinating patient care including patient treatments.

Required:

  • Graduate of an accredited and/or approved nurse practitioner program
  • Current license as a Nurse Practitioner in the Commonwealth of Massachusetts
  • Background in acute care through prior advanced practice or nursing experience. Inpatient and home care experience preferred
  • Current certification in CPR
