Pay Range
: $90,000 – $95,000
This position oversees the day‑to‑day healthcare operations of the community support services and directly supervises the health care provided to residents. The Wellness Director assesses the health, functional, and psycho‑social status of residents, initiates individualized service plans, proactively manages care, and evaluates effectiveness to maximize residents' opportunity to remain in the assisted living environment.
Position Overview- Manage staffing for the Assisted Living/Memory Care community by interviewing, hiring, training, and supervising the Assisted Living Coordinator, Reflections Coordinator, and MCMs.
- Manage residents’ health care, including dissemination of information to families, staff, physicians, and third‑party providers.
- Perform duties in accordance with established methods and techniques and in conformance with recognized standards.
- Conduct on‑site assessments of residents admitted to hospitals or skilled nursing facilities and maintain contact with the resident’s family and discharge planner.
- Complete pre‑admission assessments for each resident and update every six months or as needed.
- Initiate resident‑negotiated service plans per physician and community policy and provide instruction to residents and families.
- Perform ongoing assessments of residents’ physical and psycho‑social needs and coordinate with other departments to assure proactive quality care.
- Order medications and oversee residents’ medication schedules; audit medication records and carts twice weekly and ensure proper documentation.
- Perform on‑call duties for staffing and scheduling, and work as MCM when other MCMs are unavailable; provide coverage in emergencies to ensure medication assistance.
- Ensure all infection control guidelines are met.
- Maintain knowledge of Assisted Living regulations, medication administration regulations, and occurrence reporting.

Love, Kindness, Honesty, Goodness, Fairness, Respect. - Current nursing license (LPN or RN) and First Aid certification.
- Minimum of 1 year experience working with the elderly in an assisted living, skilled nursing, or hospital environment.
- Must complete continuing education as required by law and attend in‑service trainings.
- Knowledge of procedures and techniques for administering treatments and providing resident‑care services; understanding of universal precautions.
- Ability to read and interpret safety rules, operating instructions, procurement manuals, and other documents.
- Excellent communication skills in English, both written and oral.
- Basic typing skills and working knowledge of personal computing and word processing software.
