Intermountain Health is seeking an experienced Nurse Practitioner or Physician Assistant for the Heart Failure Clinic in Wheat Ridge, CO. You will work with a Heart Failure Cardiologist to advance a premier program and provide care at the Wheat Ridge Cardiology clinic and Lutheran Hospital.
You will manage stage 3–4 heart failure patients, ensure medication adherence, monitor diet and lifestyle changes, and coordinate care across hospital and outpatient settings.
