Horizon Hospice is seeking a compassionate and dedicated Master of Social Work (MSW) professional to join our team.

Pay Per Visit Structure:
Starting at $50/visit

Position Overview:

As an MSW Social Worker, you will report directly to the Director of Clinical Services and play a vital role in supporting patients and families through the hospice journey.

Key Responsibilities

• Conduct initial psychosocial, emotional, spiritual, and bereavement assessments
• Develop and implement goal-directed interdisciplinary care plans
• Provide ongoing counseling and crisis intervention for patients and families
• Participate in IDG (Interdisciplinary Group) meetings and bereavement team activities
• Advocate for patient and family needs
• Coordinate with community resources and agencies
• Maintain accurate, timely documentation of all patient interactions
• Support patients and families with end-of-life care and bereavement needs

• Ability to work independently and make sound clinical judgments
• Strong interpersonal and communication skills
• Emotional resilience and ability to manage stress in sensitive situations
• Flexibility, innovation, and compassion in care delivery

Physical & Working Conditions

• Minimal physical exertion (walking, reaching, lifting assistance up to 100 lbs)
• Exposure to clinical environments (including bodily fluids and varying conditions)
• Ability to adapt to temperature and environmental changes

Transportation Requirements

• Valid driver's license
• Reliable transportation and auto liability insurance
