Overview

This role supports a Mobile Physical Therapy Team delivering outpatient physical therapy directly in patients’ homes and may include telehealth services.

Responsibilities
  • Demonstrates the four NM values of teamwork, patients first, integrity, and excellence in daily work and interactions.
  • Maintains a friendly, approachable, professional demeanor and appearance; provides accurate information and timely updates to patients and customers; resolves questions and concerns promptly or directs to the appropriate resource.
  • Uses effective service recovery skills to solve problems or service breakdowns when they occur.
  • Maintains primary responsibility over patient plan of care through timely and consistent collaboration with treating PT/PTA(s).
  • Collaborates with others across departments; communicates effectively, respects diverse opinions, and acknowledges contributions.
  • Uses organizational resources efficiently and manages work schedule to complete tasks on time.
  • Contributes to continuous improvement and participates in efforts to reduce costs, streamline processes, and grow services.
  • Adapts treatment creatively to special circumstances with team feedback and documentation; demonstrates essential skill competencies in evaluation, treatment modalities, equipment, and protocols.
  • Maintains responsibility over the course of physical therapist patient management from initial contact to discharge for functional progress, safety, and outcomes; documents in EMR and coordinates with treating PTA(s).
  • Completes documentation including evaluations, progress notes, discharge summaries, and notes that support payer interpretation and reimbursement.
  • Provides educational materials appropriate to the patient’s level of understanding and motivation; participates in committees, department processes, and special projects.
  • Develops professional relationships with healthcare teams and participates in internal/external marketing as per department expectations.
  • Fosters a culture of respectful communication, openness to feedback, and timely acknowledgment of contributions.
  • Completes non-patient care responsibilities within established timelines and contributes to departmental education efforts (e.g., in-services, journal clubs, new hire orientation).
  • Supports student clinical or observation programs as a clinical instructor, mentor, or observation host per department expectations; maintains productivity standards and willingness to flex schedule as needed.
Qualifications

Required:

  • Bachelor’s Degree in Physical Therapy.
  • Licensed Physical Therapist in State of Illinois.
  • BLS from American Heart Association upon hire.

Preferred:

  • Master’s or Doctoral Degree in Physical Therapy.
