IHR means the hours are 'as needed' (shift coverage). Schedule is not guaranteed and will fluctuate depending on the team's needs. Availability may be needed anywhere from one shift per week to an entire week of coverage. Shifts are during the day, on weekdays.
Case Therapist IThe Case Therapist I reflects the mission, vision, and values of NM, adheres to the organization’s Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines and all other regulatory and accreditation standards.
Responsible for assessing patients, treatment planning, and providing direct patient services including individual, group, and family counseling. The Case Therapist coordinates care including communicating with the treatment team, patient, and family, documenting in medical record, and discharge planning, continuum of care.
Responsibilities- Facilitate individual and group counseling effectively, and provide family counseling as assigned to assist the family system in working toward resolution of dysfunction.
- Understand and communicate clearly, concisely, and in a motivational fashion, information pertaining to self‑help groups that assist patient in making a transition to such groups for ongoing support and problem solving.
- Understand DSM diagnosis and, through clinical contact with the patient and input from the treatment team, develop therapeutic interventions to assist patient with addressing diagnosis.
- Establish rapport with patient and their support system through techniques that encourage trust, disclosure, and behavioral changes to assist with recovery.
- Utilize feedback from supervisor and Clinical Director/Medical Director regarding performance, and keep scheduled supervisory sessions.
- Develop effective discharge planning with input from multidisciplinary team and managed care/utilization review.
- Secure appropriate releases of information and consents for next level of care; be knowledgeable about referral sources and community resources.
- Handle sudden deaths, trauma and accidents to provide support to family members. Assist with referrals for hospital patients in need of guardianship, advocacy, and physical, sexual, and elder abuse/neglect referrals.
- Communicate with patient and support system about admission, continued stay, and discharge planning regularly and document this communication in the medical record.
