RN Registered Nurse, Home Care / Home Health Case Manager

Immediately hiring an RN nurse case manager to join our team in Wheaton, IL!

Pay: $70,000 to $75,000 per year (based on experience)

• Monday through Friday, 8:30 AM to 5 PM, plus occasional weekends as needed.
• Local travel to client homes and facilities; mileage reimbursement provided.

• At least 1 year of paid RN nursing experience (3‑5 years of clinical experience with 1 year in a home health agency, primary care clinic, or health facility preferred).
• Current unrestricted RN license in Illinois.
• Valid vehicle with insurance, able to commute reliably.
• Understanding of regulatory compliance with state and federal regulations, HIPAA, and client confidentiality.

• Conduct nursing and/or needs assessment at the start of care.
• Conduct meet and greets for new field staff placements.
• Monitor and coach home nursing and home services field staff.
• Review nursing notes and coach field staff to improve quality.
• Review weekly care notes for quality of documentation and changes in patient condition requiring revisions to the plan of care.
• Perform nurse case management: contact families, coordinate care, conduct field visits—both home and facility—communicate changes in plan of care, and obtain physician’s signature approval as necessary.
• Foster trusted relationships with peers and referral sources in facilities.
• Participate in a rotating on‑call schedule.
• Administer prescribed medications and treatments per approved nursing techniques.
• Conduct field supervisory visits and perform competency reviews of field staff skills as needed.
• Perform field injections, medication set‑ups, and blood draws as time permits.
• Observe patients, record significant conditions and reactions, and notify supervisor or physician of patient’s condition and reaction to drugs, treatments, and significant incidents.
• Take and record patients’ vital signs to detect deviations from normal and assess the condition of the patient.
• Respond to life‑saving situations based on nursing standards, policies, procedures, and protocol.
• Document nursing history and physical assessment for assigned patients.
• Initiate a patient education plan according to the patient’s individualized needs as prescribed by the physician and/or agency policy, including patient and family instruction.
• Work with the physician and family to ensure that discharge planning is communicated to the patient.
