Cardiac Rehab Nurse – Overview

The position coordinates the care of Phase I Cardiac Rehab patients at the hospital. It is a casual position; hours per week are not guaranteed and will vary based on staffing needs. Hours are Monday, Wednesday, Friday 6:30 am‑6 pm, Tuesday and Thursday 7:30 am‑3 pm. The role covers Delnor and CDH (Wheaton) Cardiac/Pulmonary Rehab locations.

• Provide patient initial assessment & orientation.
• Develop rehabilitation plan addressing risk stratification, risk factors & exercise/functional status.
• Develop patient rehabilitation discharge goals promoting transition back to community health & exercise resources.
• Communicate rehabilitation plan & discharge to ordering physician(s).
• Provide & document patient care per department policy & external regulatory requirements.

• Bachelor of Science in Nursing.
• BLS.
• ACLS (within 6 months of hire).
• Current state of IL RN License in good standing.
• Basic computer knowledge.
• Ability to communicate effectively in English, both verbally and in writing.
• Excellent customer service skills.

• One year of experience in pertinent clinical specialty.
