RN Case Manager

The RN Case Manager is responsible for overseeing a caseload of home-health patients, completing skilled nursing visits, developing individualized care plans, coordinating interdisciplinary services, and ensuring compliance with state and federal regulations. This role is ideal for a nurse who thrives in a flexible, autonomous environment and enjoys building meaningful relationships with clients and families.

• Complete initial and ongoing in-home assessments, identifying physical, psychosocial, and environmental needs.
• Develop, implement, and update individualized plans of care in collaboration with physicians and the care team.
• Provide skilled nursing services, including wound care, medication management, disease-specific interventions, and patient education.
• Communicate changes in condition to physicians and obtain updated orders as needed.
• Document all care accurately and timely in the EMR, including OASIS assessments when applicable.
• Participate in interdisciplinary case conferences to ensure coordinated, high-quality care.
• Educate patients and caregivers on disease processes, safety, medications, and self-management strategies.
• Support safe discharge planning and continuity of care.

• Active Registered Nurse (RN) license in the state of Illinois
• Minimum 1 year of nursing experience; home-health experience preferred.
• Strong clinical judgment and critical-thinking skills.
• Excellent communication, organization, and documentation abilities.
• Valid driver's license and reliable transportation.

Salary: $55,000 – $60,000 per year
