We are looking for a new team member to join our intensive home visiting services team as a Home Visitor/Case Manager for our Healthy Families Program. As a home visitor/case manager, you will be responsible for building and sustaining a relationship with a caseload of diverse families who are pregnant or parents of newborns through the child's earliest years. This position requires travel throughout DuPage County.

  • Works with and provides services to young children and their families, by providing weekly home visits at the start of services.
  • Outreaches families and builds and establishes trusting, long term (3-5 year) relationships with families.
  • Manages a client caseload consistent with department productivity standards.
  • Meets with clients in their homes and in the community throughout DuPage County.
  • Maintains communication with clients via phone, text, and video visits.
  • Assess family needs.
  • Conducts assessments and screenings including parent interview about the family's story and screenings for developmental functioning, depression, substance use, and family violence.
  • Connects families with appropriate resources.

Bachelor’s degree from an accredited college or university preferred, with a preference for coursework in Early Childhood, Social Work, Human Services, or a related field. Designated positions may be required to be bilingual, Spanish speaking, based on the population served. Must have dependable transportation, valid driver’s license. Attendance at our on-site paid HR new hire orientation in Wheaton is required.
