Overview
Schedule: Sunday-Thursday 8a-4p. This position prioritizes engaging families experiencing homelessness, often with individuals experiencing behavioral health challenges that may include mental illness, substance use, and/or intellectual or developmental disabilities. This role delivers street outreach to coordinate immediate shelter, provide case management, crisis intervention, and care coordination services to promote stability, recovery, and independent living.
Responsibilities- Conduct proactive street outreach and respond to community referrals to engage unhoused individuals and families.
- Coordinate immediate shelter placements for families, including transportation, required documentation, and providing basic needs.
- Develop and implement individualized care/goal plans focused on housing stability, wellness, and independence.
- Provide crisis intervention, stabilization, and immediate support in community-based settings.
- Deliver case management, supportive counseling, and community support services based on client needs.
- Complete and document Continuum of Care (CoC) Assessments for Family Shelter Service, as requested.
- Coordinate partnerships with school districts, libraries, and other agencies to provide on-site outreach and engagement to potential clients, and to receive referrals for families who may need diversion or sheltering services.
- Assist clients in accessing shelter, permanent housing, and critical resources, including healthcare, benefits and social services.
- Support clients in developing independent living skills, coping strategies, and symptom management.
- Coordinate care with internal teams and external providers to ensure continuity and quality of services.
- Promote recovery and community integration through connection to natural supports, such as vocational, educational, social, and recreational opportunities.
- Perform on-site duties at agency shelter or sheltering sites, including the Access Center, to provide screening and case management services.
- Communicate assessment information, care plans, and other relevant information to team members to ensure consistency in client interventions.
- Document all services and data elements while adhering to accuracy and timeliness standards.
- Participate in team meetings, supervision, and ongoing training.
- Contribute to community education and relationship-building efforts to support individuals experiencing homelessness.
- Perform other duties as assigned by supervisor.

- Experience in person-centered, trauma-informed interventions.
- Ability to be creative, think conceptually, plan, analyze, and apply problem solving skills.
- Effective written and oral communication skills, including crisis intervention skills.
- Ability to work independently and work as part of a team.
- Computer literacy.
- All people that can successfully perform the role are encouraged to apply. Preferred:
Bachelor’s degree in social work, human development, psychology, or related field with experience serving individuals with behavioral health, intellectual, and/or developmental needs. - Knowledge of best practices in Motivational Interviewing, Harm Reduction, Housing First, life skills teaching, and areas of recovery are preferred.
- Willing to work a flexible schedule including evenings, weekends, and holidays.
- Provide own transportation.
- Must possess valid Illinois Drivers' License.
