This full‑time hybrid Case Management Nurse role is based in Wheeling, IL, with opportunities for limited work‑from‑home flexibility. Responsibilities include managing patient cases, coordinating care plans, developing discharge plans, and performing utilization reviews. The nurse will collaborate with interdisciplinary teams to ensure patients and their families receive appropriate, high‑quality care tailored to their needs.

• Strong nursing knowledge and experience in providing medical care and support
• Expertise in case management and discharge planning to ensure comprehensive and seamless patient care
• Proficiency in utilization management practices and procedures
• Effective communication, organization, and time‑management skills
• Current Registered Nurse (RN) license in the state of Illinois
• Experience in hospice or palliative care is preferred
