Clinical Social Worker - Wheeling Adult Primary Care

Welcome! We’re excited you’re considering an opportunity with us. This role collaborates with Primary Care Providers and a collaborating Psychiatrist to provide comprehensive treatment and care management for behavioral health. The Clinical Social Worker carries a caseload and routinely provides clinical interventions, completes validated assessments, offers psychoeducation, and assists in the treatment and diagnosis of behavioral health conditions.

Minimum Qualifications
  • Master's Degree in Social Work or related field
  • Current license as required by state board where services will be provided:
    • MD: Licensed Master Social Worker (LMSW), Licensed Certified Social Worker (LCSW) or Licensed Certified Social Worker – Clinical (LCSW‑C) through the Maryland Board of Social Work
    • OH: Licensed Social Worker (LSW) or Licensed Independent Social Worker (LISW) through the Ohio Board of Social Work
    • PA: Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) by the Pennsylvania Board of Social Workers, Marriage and Family Therapists and Professional Counselors
    • WV: Licensed Graduate Social Worker (LGSW), Licensed Certified Social Worker (LCSW), or Licensed Independent Clinical Social Worker (LICSW) through the West Virginia Board of Social Work
  • Two (2) years of direct clinical experience with behavioral health

Core Duties and Responsibilities
  • Provides psychosocial assessments of patients and families to identify emotional, social, and environmental strengths and problems related to diagnosis, illness, treatment, and life situation.
  • Collaborates with all members of the multidisciplinary team to provide comprehensive mental health support for designated caseload.
  • Monitors patient progress using validated scales and clinical judgment; intervenes as necessary to ensure the plan of care is patient‑focused, high quality, efficient, and cost effective.
  • Provides brief interventions, psychoeducation, and direct care management related to patients’ diagnosis, illness, treatment, and life situation.
  • Meets with patients and/or families to assess needs, develop individualized care plans, and arrange resources in collaboration with the care team.
  • Forms, develops, and implements comprehensive psychosocial treatment plans with appropriate interventions (e.g., crisis intervention, brief individual, marital, family and/or group therapies, grief and bereavement work).
  • Assists with screening, identification, diagnosis, management, and treatment of victims of abuse, neglect, domestic violence, rape, etc.; provides consultative services to health‑care team members as needed; maintains working knowledge of relevant medical/legal issues (e.g., advance directives, child and elder abuse).
  • Works closely with Primary Care Provider and Collaborating Psychiatrist on identified patients to diagnose, treat, and manage mental health and/or substance abuse problems.
  • Documents assessments, plans, interactions, and interventions according to departmental, hospital, and health‑system guidelines and standards.
  • Leverages hospital, medical center, and health‑system resources to meet patient care needs.
  • Maintains knowledge of therapeutic techniques and interventions; stays aware of available resources.
  • Stays current with social work and health‑care developments and seeks to enhance job‑related knowledge.
  • Provides teaching and training for students, staff, and faculty from social work and other disciplines; contributes to the academic mission of the medical center and health system through committees, conferences, publications, teaching, and research.
  • Exhibits consistent professional behavior.
  • Attends and participates in staff, committee, department, and other administrative meetings.
