Center Well Home Health is seeking a Home Health Physical Therapist to help patients regain strength and independence in the comfort of their homes. Reporting to the Clinical Manager, you will plan and deliver therapy focused on functional improvement and quality of life.
You will assess patients, develop rehab plans, supervise PTAs and home health aides, document progress, and communicate with physicians and families. A valid PT license and prior home health experience are preferred.
