What You’ll Do:

Develop comprehensive treatment plans and ensure appropriate placement for patients upon discharge.
Conduct thorough social assessments during both admission and discharge to support each patient’s recovery journey.
Research and connect patients with social programs and community resources to help them achieve successful, independent living.
Collaborate with families to provide guidance on how they can support loved ones coping with the effects of mental illness.
Be a key part of a treatment team, contributing to the holistic care of each patient and ensuring smooth transitions throughout their care.
Additional duties as needed to support hospital operations and patient care.

Requirements:

This is a tobacco free facility. Chosen applicants must successfully complete a chemical urinalysis and breath analysis drug screening test. All employees shall be subject to drug testing for probable cause as set forth in Agency and Bureau policies. If needed, you must be able to work holidays and weekends. Mandatory overtime, as necessary, is an essential function of this position.

Candidates must successfully pass a background screening.

Education (Must qualify based on one of the following):

Bachelor's degree or higher in Social Work from an accredited college or university.

Bachelor's degree in Sociology, Psychology, Counseling, or Criminal Justice from an accredited college or university. Please provide documentation of the approval with your application.

Bachelor's degree in a related field, as determined and approved by the West Virginia Board of Social Work. Please provide documentation of the approval with your application.

Licensure/Certification (Must qualify based on one of the following):

Applicants selected for this classification must obtain a Social Work License or Temporary Social Work License prior to beginning employment.

Provide a valid eligibility determination from the West Virginia Board of Social Work for a Provisional Social Work License.

Note:

Individuals must first be determined eligible for a Provisional License and should request an Eligibility Application. An eligibility determination is not a license but indicates to potential employers that the individual is eligible for a Provisional License if hired. Upon hiring, the individual must obtain an approved licensing supervisor and return the Provisional License Agreement to the Board. After approval and payment of the license fee, the individual may be issued a Provisional License.
