Ardor Health Solutions is seeking a Physical Therapist for a Skilled Nursing Facility contract in Shepherdstown, West Virginia
.

This is a great opportunity for a PT who enjoys working with an adult and geriatric population
, helping patients regain strength, mobility, balance, and independence following hospitalization, illness, injury, or surgery.

• Skilled Nursing / Rehab setting
• Adult and geriatric population
• Short-term rehabilitation and longer-term skilled needs
• PT evaluations and individualized plans of care
• Gait and transfer training
• Balance and fall-prevention interventions
• Strengthening and therapeutic exercise
• Functional mobility training
• Patient and caregiver education
• Discharge planning
• Interdisciplinary collaboration with nursing, OT, SLP, physicians, and facility staff

• Graduate of an accredited Physical Therapy program
• Active West Virginia PT license or ability to obtain prior to start
• Current BLS certification
• SNF/geriatric experience preferred
• Strong evaluation, documentation, and communication skills
