Job Summary
The home health nurse provides direct patient care in the patient's home environment as part of a multidisciplinary team that includes PT, OT, speech, social work, and home health techs. The nurse evaluates patients and uses professional judgment to design an individualized plan of care that best meets the patient's needs, supervises care provided by aides, documents care electronically, and collaborates with the clinical team via email and laptop communication.
Responsibilities- Provide direct, one‑on‑one patient care in the home setting.
- Develop individualized plans of care based on patient evaluation.
- Supervise and coordinate care provided by aides.
- Maintain electronic documentation and communicate with the clinical team.
- Participate in performance‑improvement activities, policy and procedure review, and research.
- Begin work after a 6‑10 week orientation with a seasoned preceptor.
- Travel to patients in Jefferson, Berkeley, and Morgan counties.
- Licensed Practical Nurse (LPN) with an active nursing license in West Virginia (Virginia license may be required for certain assignments).
- Knowledgeable in the cognitive, physical, emotional, and chronological stages of human growth and development.
- Ability to assist the RN in age‑appropriate assessment, care, and treatment of assigned patients.
- Reliable phone communication with hospital and home health.
- Ability to transfer computer data and use a laptop for documentation.
- Preferred: previous home health experience.
- BLS Certification (Basic Life Support) – American Heart Association (AHA) required.
- Healthcare Provider (HCP) – AHA approved required.
- Current driver’s license and reliable transportation.
- New hires must have AHA certification prior to completion of orientation.
- Non‑exempt, Physical Demands: 20%.
