In-Home Nurse Practitioner or Physician Assistant Per Diem - Huntington

Job Overview

We are seeking a Per Diem Clinician (Nurse Practitioner or Physician Assistant) to provide in‑home health evaluations using an intuitive clinical workflow. Each visit lasts approximately 45 minutes and includes medication and medical history review, physical examination, and optional diagnostic tests (e.g., diabetic eye exam, spirometry). Clinicians are not required to prescribe or order lab tests.

• Conduct in‑home health evaluations and build trusted relationships.
• Deliver medication and medical history review, physical exam, and diagnostic tests as ordered.
• Use an iPad and clinical workflow tool for each evaluation.
• Provide all Signify Health services, including diagnostic and preventive services such as spirometry, DEE, and bone density screening.
• Travel locally and within licensed states as needed; conduct virtual visits when required.

The primary workplace is the private residence of health‑plan members, which may contain pets, secondhand smoke, or other allergens. Candidates must be able to work safely in such environments.

• Active, unrestricted license(s) in the covered area(s).
• Board certification (required).
• Multi‑state license(s) (preferred).

Hourly base rate with additional incentive structure (e.g., per visit completed). Malpractice insurance coverage provided during Signify Health‑related activities. Supplies and other perks available.

Pay range: $37.66 – $81.11 per hour, depending on experience, education, geography and other factors.

Anticipated weekly hours: 12.
