Per Diem Clinician (Nurse Practitioner or Physician Assistant)

Signify Health, part of CVS Health, is seeking a Per Diem Clinician to provide in-home health evaluations and a variety of diagnostic services. The role involves traveling to members’ homes, performing physical assessments, reviewing medical history, and offering preventive services.

• Conduct in-home health evaluations lasting approximately 45 minutes, including medication and medical history review, physical evaluation, and as ordered, diagnostic tests (e.g., diabetic eye exams, spirometry).
• Use an iPad and an intuitive clinical workflow for each evaluation; do not prescribe medication, order lab tests, or alter existing treatment regimens.
• Travel locally and within licensed states as required; conduct virtual visits when needed.
• Provide Diagnostic & Preventive Services products (e.g., spirometry, DEE) as indicated.

• Active, unrestricted license(s) in the coverage area(s).
• Board certification required.
• Multi‑state licenses preferred.
• Eligible to work in Cumberland and surrounding areas.

• Hourly base rate ranging from $37.66 to $81.11, dependent on experience, education, and geography.
• Part‑time, paying 12 anticipated weekly hours.
• Per diem employees receive hourly base rate plus additional incentive structure per visit completed.
