• Assesses, plans, implements, and evaluates patient care to achieve optimal outcomes for patients of all ages.
• Performs age‑specific admission and ongoing assessments, documents findings, and develops individualized plans of care.
• Collaborates with the healthcare team, patients, and families to implement interdisciplinary care plans and revise them as patient status changes.
• Administers treatments and medications accurately, documenting each intervention in electronic medical records.
• Identifies and addresses patient and family educational needs, incorporating them into the care plan.
• Utilizes critical thinking, delegation, and evidence‑based practices to support quality patient care and performance improvement.
• Plans strategies to reduce length of stay, turnaround time, and resource consumption, while improving patient outcomes.
• Serves as a clinical role model for coworkers, students, and physicians.
• Manages resources, establishes priorities, and modifies the environment to meet patient care needs.
• Participates in orientation of new employees and supports team communication across Bedminster and the Lexington area.

• Graduate of a professional nursing school.
• Minimum one year of experience in hospice, home health, oncology, or medical‑surgical nursing.
• Currently licensed as a Registered Nurse in the Commonwealth of Virginia.
• For home health, valid Virginia driver’s license and successful completion of a criminal record check.
• AHA BLS‑HCP certification required.
• Successful completion of orientation and competency requirements.
• Keyboarding skills for automated computer systems, including electronic medical records.
• Ability to meet essential job functions and manage physical demands such as lifting, standing, and reaching.
