Registered Nurse - LTC​/SNF

Job Description

Job Summary:

Seeking a dedicated Registered Nurse to provide high-quality care in a skilled nursing facility. The RN will support patients with diverse needs, including acute and long-term care, ensuring optimal health outcomes and patient safety.

Responsibilities:

• Assess and monitor residents' health status, responding promptly to changes.
• Administer medications and treatments per physician orders, including IVs and PICC lines.
• Collaborate with interdisciplinary team members such as PT, OT, SLP, and social services.
• Document patient care accurately in EMR and adhere to facility protocols.
• Educate residents and families regarding health conditions, treatments, and discharge plans.
• Lead and participate in team conferences for patient care planning and updates.

Qualifications:

• Valid RN license with relevant state board approval.
• Current CPR certification.
• Prior experience with IV therapy, PICC lines, mediports, and medication administration.
• Knowledge of long-term and skilled nursing care procedures.
• Ability to work collaboratively within a multidisciplinary team.
• Strong clinical assessment and documentation skills.
