- Evaluate patients in their homes and establish individualized treatment plans.
- Improve mobility, balance, strength, endurance, and functional independence.
- Recommend adaptive equipment and home safety modifications when appropriate.
- Educate patients and caregivers to maximize long-term outcomes.
- Collaborate with physicians, nurses, occupational therapists, speech therapists, and the interdisciplinary care team.
- Maintain timely and accurate clinical documentation.
- Active Minnesota Physical Therapist (PT) license.
- Strong clinical reasoning and patient-centered approach.
- Excellent communication and relationship-building skills.
- Ability to work independently while managing a flexible field schedule.
- Valid driver's license and reliable transportation.
