Schedule:
Full-Time with On-Call and Weekend Rotation
Compensation: $75,000 to $100,000 per year plus a $4,000 Sign-On Bonus

What Could Your Day Look Like?
You'll travel throughout the St. Paul area , completing skilled nursing visits that may include:
- Wound care and wound assessment
- Medication reconciliation and management
- Injections and other skilled nursing procedures
- Monitoring acute and chronic medical conditions
- Patient and caregiver education
- Assessing progress following hospitalization or rehabilitation
- Identifying and communicating changes in condition
- Coordinating with physicians and other healthcare providers
- Collaborating with PT, OT, SLP, social work, and interdisciplinary team members
- Helping prevent avoidable emergency department visits and rehospitalizations
- Completing timely documentation in Homecare Homebase (HCHB)
