Private Duty LPN

Role: Home and Community Based Services Licensed Practical Nurse, providing direct nursing care to acutely or chronically ill patients in their homes.

• Deliver high-quality clinical services within legal scope, following the plan of care and infection control protocols.
• Thoroughly document patient visits in electronic medical records and visit verification systems.
• Participate in care coordination by reporting patient needs, attending case conferences, and communicating with patients, families, physicians, and team members.
• Adapt to diverse home environments, ensuring equitable care regardless of client background.

• Current, unrestricted LPN license in the practice state.
• Valid driver’s license, vehicle insurance, or access to reliable transportation.
• Preferred: CPR certification, flexibility, multitasking ability, independence, and strong communication and organizational skills.
