Private Duty LPN

We are seeking a Home and Community Based Services Licensed Practical Nurse to provide direct nursing care in patients' homes, including recording clinical notes and coordinating care activities to ensure high-quality patient outcomes.

Under supervision of a Registered Nurse or designated supervisor, responsibilities include delivering clinical services within the scope of practice, documenting visits accurately in electronic records, and participating in care coordination by communicating patient needs and changes in condition.

Qualifications include current LPN licensure in the practice state, a valid driver's license, and reliable transportation. Preferred skills encompass CPR certification, effective communication, organizational abilities, and independence
. Ability to work flexibly, manage multiple tasks, and serve diverse populations is essential.
