INTAKE COORDINATOR
VNS Westchester, founded in 1901, is Westchester's premier and only certified home health care agency. We are seeking an Intake Coordinator to review and process referrals.
As the Intake Coordinator, you will screen referrals and organize them for skilled home care visits, capturing correct patient demographics and clinical admission information for entry into the electronic medical record. You will review insurance coverage and set up accurate financial coverage through insurance eligibility investigation. You will connect with professionals, caregivers, or patients via electronic platforms, phone, fax or email to obtain all information needed to produce a start‑of‑care visit.

Responsibilities- Process patient referrals in the electronic medical record; review and enter demographics, clinical information and financial reimbursement information.
- Review clinical documentation to ensure the patient meets criteria for Certified Home Health Agency services.
- Schedule Start‑of‑Care dates with patients, add dates to the electronic medical record and the intake board.
- Verify eligibility and benefits for all Medicare, Medicaid, Commercial and Managed Care insurances using websites.
- Verify enrollment of new physicians in PECOS and OPRA databases; contact doctors as needed to facilitate obtaining documents and orders for new referrals.
- Screen incoming referrals and accept or decline requests based on home‑care appropriateness.
- Contact patients, caregivers or referral sources to obtain information needed for admission.
- Collaborate with other intake staff to gather all information for each referral.
- Work within the weekend rotation schedule to support the department on weekends and holidays.
- Cultivate relationships with medical practices that refer patients.
- Perform other duties as assigned.
- Proficient in data entry and navigation of an EMR.
- Strong experience with insurance referral, eligibility and authorization processes.
- Understanding of medical terminology and treatment plans.
- Excellent customer service and follow‑up skills.
- Strong organizational skills; case management experience is a plus.
Salary: $26.50 to $29.50 per hour
Schedule: Sunday through Thursday and Monday through Friday shifts available.
