** Job Summary
** The team leader is responsible for all aspects of clinical management, and for staff mentoring and supervision for Program/service delivery for a specific patient population. In collaboration with the Program Director and SVP of Post Acute Operations, as appropriate, the team leader ensures the efficient and effective operation of therapy services, participates in program development and provides leadership and education to mentor staff and to meet patient needs.

This position reports to the SVP of Post Acute Operations.

** Essential Functions
*** Clinical Management/Operational Responsibilities:
* Coordinates the day-to-day scheduling and coverage of patients
* Conducts regular team meeting with the goals of team building, dissemination of information, quality improvement
* Responsible for direct patient care
* Demonstrates clinical competencies with a variety of patient populations
* With Program Directors and AVP, ensures compliance with the “3 Hour Rule”
* Staff Supervision, Education, and Training:
* Provides staff orientation/competency assessments
* Provides informal and ongoing mentoring and feedback
* In collaboration with the Program Director, participates in completion of formal performance evaluations for all team members in accordance with established policies.
* Participates in in-service planning and execution
* Leadership/Mentoring:
* Promotes positive problem solving and teamwork
* Fosters an environment of respect and open communication
* Serves as a resource person for staff
* Communicates clearly and concisely, with effective use of verbal and non-verbal skills
* Enhances professional growth and development through, i.e., continuing education and professional affiliations
* Focuses on patient safety, patient satisfaction, quality, and customer service
* Collaborates with Medicine, Nursing, and other departments as needed
* Creates and maintains positive relationships with patients, families, visitors, medical staff, and all hospital departments.
* Program Development:
* Assists and actively participates in program development, with the Program Director, including, but not limited to: marketing, identification of equipment/technologies, program development activities within/outside the hospital, budget planning
* Participates in the program’s performance improvements initiatives
* Assists the Program Director in the development, and periodic review, of policies and program needs.
* Performs other duties as requested by Program Director
* Practices Standard Precautions. Follows regulations, policies and guidelines on the prevention of infections; does not eat or drink in patient care areas.
* Maintains confidentiality of information; abides by HIPAA regulations.
* Maintains a professional image and follows Hospital’s policy on uniforms; consistently wears hospital .
* Demonstrates reliable, on-time attendance, which is an essential function of all jobs at Burke.
* Completes mandatory annual learning (Learning Harbor) within deadline.
* Personally adheres to Burke C.A.R.E.S. philosophy by demonstrating compassion, accountability, respect and exceeding expectations through service excellence.

** Qualifications
* ** BS Required or MS Required
* 4-6 years experience required (2.5 years with diagnostic group and 1.5 years progressive managerial experience preferred)
* Works with staff on the specific program, patients, families and interdisciplinary team, physicians
* Must be able to perform dependent assistance transfers
* Must demonstrate exemplary customer service
* Must demonstrate decision making skills, a commitment to excellence, and problem solving skills
* Ability to cultivate teamwork
* Must have NY State License in Physical, Occupational, or Speech…
