Position Overview
Nurse America is seeking a dedicated Licensed Practical Nurse (LPN) to join a supportive assisted living and memory care community in White Plains, NY. This position focuses on day-to-day resident care, including medication administration, monitoring health conditions, and assisting with care plans. The role offers a steady and team-oriented environment where you can build strong connections with residents and provide personalized care.
You will work alongside RNs and CNAs to ensure residents receive consistent support in a calm and organized setting. This opportunity is ideal for LPNs looking for a stable role outside of hospital settings. Flexible per diem and full-time schedules are available.
Schedule
- Flexible scheduling including evenings and weekends
Pay Rates:
- LPN (Full-Time): $35 – $40/hr

Key Responsibilities
- Administer medications and assist residents with self-administration
- Monitor resident health status and report changes to RN/physician
- Maintain accurate medication records and documentation
- Support physician orders and treatment plans
- Assist with resident assessments and care planning
- Coordinate appointments and follow-ups as needed
- Respond to emergencies and provide basic nursing care
- Ensure compliance with safety and regulatory standards
- Provide compassionate, resident-centered care
Qualifications
- Active LPN license (NY State)
- Minimum 2–3 years of experience (Assisted Living, LTC, or Memory Care preferred)
- Strong knowledge of medication administration
- Experience with geriatric and dementia patients preferred
- Good communication and interpersonal skills
- Ability to work flexible schedules
- Basic computer skills
- Clear background check
