Location: City of White Plains
Affirmed Home Care is seeking a compassionate and experienced Registered Nurse (RN) Care Manager for pediatric home care in Westchester County, NY. This per diem, field-based role allows flexibility and autonomy in delivering exceptional care to pediatric clients.
Responsibilities include conducting in-home nursing assessments, developing care plans, and collaborating with interdisciplinary teams. Qualified candidates must have 2+ years of pediatric nursing experience and a valid New York State RN license.
Competitive pay rates, including $150 per assessment and $55/hour, are offered alongside additional benefits and a supportive clinical leadership team.
