Affirmed Home Care, Inc. is looking for a Registered Nurse (RN) Care Manager to provide exceptional pediatric home care across Westchester County, NY. This per diem position allows for flexibility and autonomy while delivering one-on-one care to our young clients and their families.
As an RN Care Manager, you will be responsible for creating personalized care plans, conducting nursing assessments, and leading interdisciplinary collaboration to ensure client safety and comfort. Ideal candidates have a strong background in pediatric care and must hold an active New York State RN License.
