Job Overview – RN Care Manager
Compensation: $125,000 – $160,000/year
Location:
Westchester County, NY
Schedule:
Monday to Friday, 9:00 AM – 5:00 PM
TAG Med Staffing is hiring an RN Care Manager in Westchester County, NY for our client, supporting hospital discharge planning, care coordination, utilization review, and transitional care services within an acute care setting. This opportunity will work closely with interdisciplinary healthcare teams to assess patient needs, coordinate post-acute services, reduce readmissions, and ensure safe and effective discharge planning. The ideal candidate will bring strong hospital-based care management experience, discharge planning expertise, and a Registered Nurse license in New York.
Responsibilities as the RN Care Manager:- Care Management Assessments: Complete comprehensive patient assessments, identify discharge planning needs, and evaluate risk factors that may impact transitions of care.
- Discharge Planning: Coordinate post-acute services, patient education, caregiver support, and community resources to facilitate safe discharges and prevent readmissions.
- Care Coordination: Collaborate with physicians, nursing staff, payers, and interdisciplinary teams to develop and implement patient-centered care plans.
- Utilization Review: Perform utilization review activities, payer communications, admission reviews, and documentation to support appropriate levels of care.
- Transitional Care Management: Monitor patient progress throughout hospitalization and coordinate transitions across care settings to optimize outcomes.
- Documentation & Compliance: Maintain accurate documentation and ensure compliance with regulatory requirements, organizational policies, and care management standards.
- Education: Bachelor of Science in Nursing (BSN) is required.
- Licensure: Active New York State Registered Nurse (RN) license is required.
- Experience: Minimum of 2 years of hospital discharge planning or hospital care management experience is required, and prior utilization review experience is required.
- Industry Knowledge: Knowledge of acute care operations, healthcare reimbursement, payer guidelines, transitional care, and community-based resources is required.
- Technical
Skills:
Experience utilizing care management systems, utilization review criteria, and Microsoft Office applications is required. - Skills & Attributes: Strong communication, critical thinking, organizational, and multitasking skills with the ability to coordinate complex patient discharges and collaborate effectively across multidisciplinary teams are required.
