In-Home Health - Nurse Practitioner or Physician Assistant; Part Time -Colorado Springs

Signify Health, part of CVS Health®, is seeking a part‑time Clinician (Nurse Practitioner or Physician Assistant) to provide in‑home health evaluations. Each visit lasts about 45 minutes and includes a medication and medical history review, physical evaluation, and, if ordered by the health plan, additional diagnostic tests such as diabetic eye exams or spirometry.

• Conduct in‑home health evaluations using an iPad and an intuitive clinical workflow.
• Review medication histories and perform physical examinations.
• Order and perform diagnostic and preventive services (e.g., spirometry, diabetic eye exams) as instructed by the health plan.
• Provide education and answer health questions during visits.
• Conduct virtual visits when required.
• Travel locally and within licensed states to visit millions of people nationwide.
• Maintain patient confidentiality and adhere to safety protocols in diverse home environments.

As an in‑home clinician, the primary workplace is the private residence of health plan members. Candidates must be comfortable and safe performing duties in homes that may contain pets, secondhand smoke, and other potential allergens.

• Active, unrestricted license(s) in the coverage area(s).
• Board certification (required).
• Multi‑state licenses (preferred).
• Ability to cover Colorado Springs and El Pasto County.
• Licensed Nurse Practitioner or Physician Assistant.

Expected weekly hours: 29.5 hours.

Time type:
Part‑time.

Pay range: $41.85 – $90.13 per hour (dependent on experience, education, and geography).
