This Part‑time Clinician (Nurse Practitioner or Physician Assistant) position provides In‑Home Health Evaluations, engaging with people in the comfort of their own homes to bridge gaps in care.

• Conduct 45‑minute in‑home visits, including medication and medical history review, physical evaluation, and additional diagnostic tests when ordered by the member’s health plan.
• Use an iPad and an intuitive clinical workflow for each evaluation; not required to prescribe medicine, order lab tests, or alter current treatment regimens.
• Bring empathy and professionalism into every visit, building trusted relationships and providing personalized care.
• Assist in connecting members nationwide with appropriate care providers and services.
• Be flexible to travel locally and within licensed states as needed and conduct virtual visits when required.
• Provide all Signify Health services indicated, including diagnostic and preventive services such as spirometry, diabetic eye exams, bone density screening, etc.

• Active, unrestricted license(s) in the coverage area(s).
• Board certification required; multi‑state licenses preferred.
• Ability to cover Fergus Falls, MN and surrounding area.
• Required nurse practitioner or physician assistant credentials.

• Anticipated weekly hours: 29.5 hours (part‑time).
• Pay range: $41.85 - $90.13 per hour (based on experience, education, geography, etc.).
