Registered Nurse (RN) Care Manager - Acute Care Job Description
The RN Care Manager is responsible for performing initial comprehensive assessments on patient admission, utilizing established tools to identify high-risk indicators. This role ensures that high-risk patient populations receive appropriate support services for discharge, aiming to prevent readmission. The RN Care Manager will also conduct ongoing reassessment throughout the hospital stay and perform utilization review activities according to department policy.
Responsibilities- Adhere to hospital performance standards, policies, and procedures.
- Complete comprehensive initial assessments for all patients.
- Coordinate multidisciplinary care at both intra-hospital and inter-hospital levels.
- Conduct transitional planning activities, including patient and caregiver education and aftercare arrangement.
- Communicate relevant patient information to the health care team members effectively.
- Achieve departmental goals related to length of stay reduction, readmission prevention, and satisfaction improvement.
- Participate in performance improvement activities as needed.
- Conduct utilization review activities for commercial payer reviews and clinical assessments.
- Ensure safe discharge of geriatric patients, evaluating their needs across various care settings.
- New York State Registered Nurse (RN) license is required.
- Prior discharge and clinical experience is required.
- PRI Certification to be obtained within 3 months of employment.
- A Care Management certification (i.e. CMC, CCM) is preferred.
- 3 years of clinical experience in ED, critical care, or med/surg preferred.
- Experience in case management and knowledge of healthcare financial, regulatory, and payer issues are preferred.
- Experience with Milliman/Inter Qual criteria is preferred.
- Strong computer skills are essential.
This is a 10-hour shift schedule consisting of 4 days, totaling 40 hours per week.
