Registered Nurse Care Manager

We're seeking an experienced RN Care Manager to join a collaborative acute care team focused on care coordination, utilization review, and safe discharge planning.

• MUST have hospital discharge planning and/or acute care management experience
• 2+ years of experience in an acute care hospital

• PRI Certification (or ability to obtain within 3 months of hire)
• 3+ years of Emergency Department, Critical Care, or Medical‑Surgical nursing experience
• Knowledge of healthcare financial, regulatory, and payer requirements
• Knowledge of state, local, and federal healthcare programs
• Experience using Milliman and/or Inter Qual criteria

• Complete comprehensive patient assessments and identify discharge planning needs from admission through discharge.
• Coordinate multidisciplinary care and collaborate with physicians, nurses, social workers, and community providers.
• Develop and implement safe discharge plans to reduce readmissions and improve patient outcomes.
• Perform utilization review and ensure compliance with payer and regulatory requirements.
• Educate patients and caregivers while coordinating post-acute services and community resources.
