White Plains Hospital Medical Center is seeking an RN Care Manager for day shift. You will perform initial comprehensive assessments, coordinate multidisciplinary care, and plan discharges with education and post-hospital care arrangements.
Requirements include BSN/MSN and NY RN license on hire; 1–3 years critical care/ED/med-surg experience preferred. PRI certification within 90 days is preferred; CCM/CMC certification is also favored.
