The Registered Nurse (RN) provides and coordinates the care of patients/families and significant others in the ambulatory care setting by assessing, planning and intervening in actual or potential health problems, including evaluation of the patient’s response to the care. These responsibilities also include patient/family/significant other education and health maintenance strategies. The RN is a member of a designated health care team and collaborates, coordinates and guides team members in planning patient care and achieving goals.

The Registered Nurse demonstrates knowledge of the principles of human growth and development over the life span and the ability to assess data reflective of the patient’s status and interpret the appropriate information needed to identify each patient’s requirements relative to his or her age, specific needs, and to provide the care needed as described by standards and policy and procedures.

Functions
  • Understands and adheres to the WPH Performance Standards, Policies and Behaviors.
  • Demonstrates the ability to perform clinical/technical/service/administrative tasks
  • Systematically and continuously assesses patient physical, psychosocial and spiritual health status and knowledge base to determine patient’s health care and educational needs.
  • Provides and/or delegates patient care according to the care goals.
  • Sets priorities based on patient need.
  • Triage all sick and walk-in patients
  • Establish therapeutic and supportive relationships with patients and family.
  • Acts as a resource person.
  • Coordinate client care including referral back to primary provider.
  • Assess needs and initiate referrals (VNS, Social Work).
  • Telephone triage.
  • Prescription requests, according to procedure.
  • Communicates normal and abnormal test results in collaboration with provider including appropriate follow-up.
  • Performs therapeutic treatments and procedures.
  • Assist with procedures.
  • Administer oral/IM/IV/SQ/Inhaled medications.
  • Coordinates delegates and/or provides all other patient care activities.
  • Coordinates in-patient admissions and ER referrals.
  • Acts as a patient advocate.
  • Documents according to standards of care.
  • Promotes and provides for patient safety in accordance with state and federal guidelines.
  • Participates in the department’s performance improvement activities.
  • Provides presentations to care team members.
  • Ensures all staff participates as a member of the patient care team, participates in daily huddles and attend regular scheduled practice team meetings.
  • Attends in service and continuing education programs.
  • Demonstrates a professional, courteous, and respectful attitude in dealing with patients, families and significant others.
  • Informs and educates patient/family to enable participation in their care and decision‑making.
  • Evaluates the effectiveness of interventions and patient responses and outcomes to care.
  • Collaborates with team members to make the necessary changes to patient plan of care.
  • Communicates pertinent information and changes in patient status to appropriate members of the care team and other health care providers.
  • Performs all other related duties as assigned.
Qualifications
  • BSN Required or MSN 1-3 years Prior nursing experience in health care setting preferred Preferred Knowledge of current nursing, medical and health trends and practices in rendering of patient care
